Who is eligible?

Any resident of Lake County, FL who owns a cat or dog!

Any stipulations?

Only one certificate is issued per month per household. If you need more than one certificate, allow a month between each request.

Certificates may or may not be adequate to cover the total cost of spaying or neutering your pet. It is your responsibility to call the listed veterinarians and make your decision as to which one to use.

To apply for a certificate:

1. Choose the veterinary clinic to which you will take your pet. There is a list of clinics that accept Stacks certificates under the "List of Veterinarians" tab at the top right of the page. Make sure to call the veterinary clinic to which you plan to take your pet to inquire about the cost before submitting your request as all clinics do not charge the same fee. Once you apply for a certificate, we will not be able to send your certificate to a clinic different from the one you have chosen on your initial application.

2. Fill out a certificate request form here: Certificate Request Form. Complete the form then click "submit" at the bottom of the page.

3. Once we have processed your request, we will send it straight to the veterinary clinic you have chosen. At that point, we will contact you so you can make an appointment with your veterinarian to have your animal spayed or neutered.

*Remember that certificates are only valid 60 days from the issue date so it is important that you make your appointment promptly after receiving confirmation. Also, certificates are non-transerfable so whomever plans to take the animal to the vet must be the person to apply for the certificate.

*Please allow 1-2 weeks for us to process your request. Again, we will contact you via e-mail once your certificate has been sent. Please be sure to check your "junk" or "spam" folders in case our e-mail is sent to one of those folders. Thank you.
